  If a complete team of 3 boaters and 3 non-boaters is not submitted, the Tournament Committee may, at their discretion, move boaters to non-boater positions.
  Please indicate by use of the Checkbox buttons, above, which team member is your designated Captain.
  Also, please indicate any backup boats you will be bringing if this is known at the time of submission.
  You are encouraged to re-submit this form as appropriate should you need to make changes to the team.
  Use the "Comments" area  to pass along any special instructions etc.
